HGS AxisPoint Health and Parkland Community Health Plan partner to deliver Disease Management

Hinduja Global Solutions (HGS) (listed in BSE & NSE), announced today that its subsidiary, HGS AxisPoint Health, has partnered with Parkland Community Health Plan (PCHP) to provide disease management services to approximately 195,000 Medicaid beneficiaries in a seven-county area in North Texas.

"We are excited to help improve the health and wellness of PCHP's Medicaid members," said Ramesh Gopalan, President of HGS Healthcare. "Our analytics, clinical expertise, program communications and care management will help identify and address PCHP's most impactable asthmatic and diabetic members," said Zac Fritz, CEO, HGS AxisPoint Health. "This is an opportunity to support PCHP's members by changing behavior to improve their health and reduce medical expense."

This partnership will enable PCHP to deliver elite care management by focusing on productive, dynamic engagement with targeted, impactable members. This approach meets members where they are to drive positive behavior change with the goal of achieving the triple aim - impacting cost, outcomes, and the member experience. It takes a multidisciplinary care team approach to deliver our condition management services. The team includes clinical and nonclinical professionals, including registered nurses, social workers, health coaches, enrollment specialists, a pharmacist, respiratory therapist, and a diabetes educator.

"After a comprehensive evaluation process, we selected HGS AxisPoint Health because of their National Committee for Quality Assurance (NCQA) accredited Population Health Management and Disease Management programs, scalability and experience in Texas," said Mark Clanton, Chief Medical Officer, Parkland Community Health Plan. "We knew about AxisPoint Health's Medicaid work with the Texas Health and Human Services Commission. We are looking forward to partnering with them to help our members lead healthier lives."

The disease management program is a care management service that will bring PCHP additional cost savings through lower cost of care. The programs focus intensively on a member's gaps in care that, left open, result in high-cost near-term events and poorer health outcomes.

HGS and PCHP will begin the implementation process this month, with operations slated to launch in January 2021.
